As someone who doesn't read Batman, except for the snips and snatches that others point out, I was confused at first reading at the point. (Partly I was wondering where in continuity it was and if I needed to have a synopsis of the issue--still kinda would like to know) The realization crept in slowly and was baldly confirmed by others' reviews (thank god other people are smarter than me).
How strange that Bruce seems, now more than ever, a man dressed in a costume. And yet, so evilly selfish. But Tim-as-Bat is far more scary than Batman ever was, to me. He's emptied himself of almost everything but what others required of him that he's hollow.

Can't help but hope this isn't canon, sorry.
